How to Get the Forms

If you have the Adobe Acrobat Reader, to view and print the Adobe PDF Student Forms below, simply click on the link to the appropriate form.

If you do not have the Adobe Acrobat Reader, you can download a free copy here: Adobe Acrobat Reader Download the program and follow installation the installation directions. For more information about the Adobe Acrobat Reader please refer to the site itself.

NOTE: If you are using Netscape Communicator then you can download the forms by right-clicking on the link and choosing the Save Link As from the pop up menu. In the Save Link As dialog box, select a location to save the form onto. You will then need an Acrobat viewer to view and print the form. The link for the viewer is provided below.

NOTE: If you cannot see the PDF forms in Netscape Communicator, simply press the RELOAD button on the top toolbar. If you still cannot see the PDF form, then refer to the note above.

    • This brochure provides information regarding the College of Fine and Applied Arts James Scholar Program.

  • Appeal Process [pdf]
    • FAA process to appeal a college academic decision including late dropping of classes, academic drops, and readmission decisions.
  • Standard Student Petition Form [pdf]
    • Standard petition form used by students in the College of Fine and Applied Arts for general petitions.
  • Credit/No Credit Form

    • Form used by students when electing to take a class credit/no credit. For more on credit/no credit see the FAA Student Handbook.
  • Late Course Change Form

    • Form used by students to add or drop class(es) after the deadline.
  • Concurrent Enrollment Form [pdf] for Parkland College
    • Form used by students who would like to take courses at Parkland College and the University of Illinois during the same semester. The instructions for completing the form are on the second page.
  • Foreign Language Petition for Accomodation for Students with Disabilities [pdf]

    • The FAA College Committee on Accomodations can approved a substitution to the foreign language requirement for students with relevant, documented disabilities. This form details the specific documentation that is required for this petition process.
  • Study Abroad Course Planning Forms [pdf]

    • Form used by the student and academic advisor to plan for a study abroad program. The College of Fine and Applied Arts does not recommend students wait until their senior year to participate in study abroad experiences. The Course Planning Form provides more information.
  • Guidelines for Retroactive Withdrawal [pdf]
    • Explanation of the circumstances underwhich a retroactive withdrawal may be considered along with an explanation of the necessary documentation. Document also addresses issues related to readmission if the retroactive withdrawal is granted.
  • Withdrawal Form [pdf]
    • Form used by students to withdraw from the University for the semester. Admission and Readmission.

    • Information regarding the re-admission process.
  • Admission and Readmission Chart [pdf]
    • Chart helping to identity who to talk to about admission/readmission to the UIUC and FAA.
  • Petition to Re-enter form [pdf]
    • Petition to re-enter the college.
  • GPA Recovery Calculation Sheet (Excel Spread Sheet)
    • This is an MS Excel worksheet which you can download. You can insert your UIUC grades and transfer grades into the specified boxes on the worksheet. The worksheet then creates a chart which helps you to determine the grades you would need to reach a specific cumulative GPA.
  • FAA Curriculum Acceptance Form [pdf]

    • Form to request a transfer into one FAA program from another FAA program or another UIUC college. It may also be uses as a portfolio/audition/interview report form for potential transfer students from another institution.
  • Extended Term Request Form [pdf]

    • FAA students are normally expected to complete the academic requirements for their first degree in eight semesters. Students are routinely allowed a time extension of two additional semesters to complete their degree requirement when they have changed programs. The maximum time allowed to complete a degree in Fine and Applied Arts is ten semesters unless the student’s FAA unit makes a request for an extension of time.

I (Incomplete) Grade Request

  • I Grade Information [pdf]
    • Information about the I (Incomplete) grade request.
  • I Grade Request Form [pdf]
    • Form used to request an I (Incomplete) grade. This grade requires consent from the instructor and approval of the College Office. If you have any difficulty submitting this form automatically, right-click on the file name and save the file to your hard drive. You will then be able to complete the form and submit it electronically or print it out and mail or fax it to our office.
